Iron safety, before anything else: Do not start an iron supplement without testing first. Iron is one of the few nutrients where excess is genuinely harmful. It accumulates in the body, and conditions like hereditary hemochromatosis make that accumulation dangerous. Iron supplements are also a leading cause of poisoning in young children, so if they are in your home they belong locked away and out of reach. Ask your doctor for a ferritin test and let the result guide any supplementation. Iron-rich food needs no testing. Pills are a medical decision.

Iron for women is the nutrient story that gets the least airtime and causes the most quiet damage. Not dramatic damage. The slow kind: sessions that feel harder than they should, an afternoon fog you blame on sleep, a general sense of running at eighty percent that you eventually just accept as your baseline.

Why Iron for Women Is a Different Conversation

Iron carries oxygen. That’s the job. Every cell running aerobically depends on it, which is why running low doesn’t feel like one specific symptom. It feels like everything being slightly worse.

Women of reproductive age lose iron every month through menstruation, which men simply do not. That single fact drives the entire difference in requirements: the recommended intake for premenopausal women sits at roughly 18 mg a day against 8 mg for men. More than double, from the same food supply, while typically eating fewer total calories.

Add training on top. Endurance work in particular increases iron losses through several routes, sweat and foot-strike haemolysis among them, and appetite suppression around hard sessions doesn’t help intake either.

Then there’s the group nobody counts properly. Heavy menstrual bleeding affects roughly a quarter of women, and among exercising women the surveyed numbers run higher: over half in one online survey, around a third of marathon runners, and a similar share of elite athletes. Only about a fifth of them had ever raised it with a doctor. In a Finnish cohort of women with heavy bleeding, ninety percent had ferritin below 30.

What Low Iron Actually Feels Like

Not fainting. Not pallor. Those come late, if at all, which is why low iron for women goes unrecognised for so long.

What comes first is unremarkable enough to explain away. Persistent fatigue that sleep doesn’t touch. Workouts that feel disproportionately hard for the numbers on the bar. Breathlessness on efforts you used to handle. Poor recovery between sessions. Brain fog. Irritability with no obvious cause. Cold hands and feet. Sometimes hair shedding, sometimes restless legs, sometimes a strange craving for ice.

Every one of those has ten other explanations, which is exactly why it goes unnoticed. Stress. Bad sleep. Being busy. Being thirty-eight.

The pattern worth paying attention to is several of them together, persisting for months, in a woman who menstruates. That’s not a diagnosis. It’s a reason to get tested.

The Test You Need, and the Number Nobody Explains

This is the section I’d most like women to read twice.

A standard full blood count measures haemoglobin. It tells you whether you’re anaemic. It does not tell you whether you’re iron deficient, because iron deficiency without anaemia is a distinct state that arrives first and can persist for a long time before haemoglobin drops. The fatigue, the exercise intolerance, the cognitive fuzziness: all documented in iron deficiency without anaemia. A normal full blood count does not rule it out.

The test that does is ferritin, which measures stored iron rather than circulating iron. If you ask for one thing, ask for that.

Now the complicated part. There is no single agreed threshold for what counts as deficient, and the disagreement is wide:

  • WHO and CDC: below 15 µg/L
  • NICE in the UK: below 30 µg/L
  • American Gastroenterological Association: 45 ng/mL or below
  • Physiologically-based analyses published in the Lancet Global Health and elsewhere: iron-restricted red cell production begins around 25 µg/L in premenopausal women, and around 33 µg/L after menopause

Read that list again and notice the spread. A woman with a ferritin of 20 is deficient under three of those four frameworks and comfortably normal under the first.

Why do the low thresholds persist? Partly because the WHO figure was set on expert opinion rather than on physiological measurement, which the newer research says plainly. Beyond that, one proposed explanation is that laboratory reference ranges were drawn from population samples that already contained a lot of iron-depleted people, so the “normal range” ended up encoding a depleted population as the standard. That explanation is plausible and has been put forward by researchers in the field. It has not been settled, and I’d rather tell you that than pretend the story is tidier than it is.

What to do with that: get the actual number, not the word “normal”. If your ferritin sits in the teens or twenties and you have symptoms, that is a legitimate conversation to have with your doctor rather than something to shrug off. I’m not telling you that you’re deficient. I’m telling you the threshold is contested and you’re entitled to ask where yours falls.

Heme and Non-Heme: Why Food Source Matters

Dietary iron comes in two forms and they behave completely differently.

Heme iron comes from animal flesh: red meat, poultry, fish, shellfish. Your body absorbs it well and absorption isn’t much affected by what else is on the plate.

Non-heme iron comes from plants and fortified foods: lentils, beans, tofu, spinach, fortified cereals, pumpkin seeds. Absorption is markedly lower and highly sensitive to context.

That second point is the practical one, because context is something you control.

Vitamin C substantially improves non-heme absorption. Lentils with tomato, beans with peppers, spinach with a squeeze of lemon. This is not a marginal effect and it is the single easiest change to make.

Tea, coffee, and calcium inhibit it. Polyphenols in tea and coffee bind non-heme iron, and calcium interferes with both forms. Which means the very common habit of tea with an iron-rich meal is working against you. Move the tea an hour either side and you’ve changed nothing about what you eat.

Food Sources of Iron for Women Worth Building Around

Heme sources first, since they do the most work per serving: red meat, liver if you can stomach it, oysters and mussels, sardines, poultry.

Non-heme sources that pull their weight: lentils, chickpeas, white beans, tofu, pumpkin seeds, fortified cereals, dark leafy greens.

A word on spinach, because it has an undeserved reputation. It does contain iron, but it also contains oxalates that impede absorption, so it is not the iron powerhouse the cartoon implied. Eat it because it’s good food, not because you think it’s fixing your ferritin.

When Food Isn’t Enough

Sometimes it isn’t, and that’s not a failure of discipline.

If your losses are high, monthly bleeding is heavy, and your absorption is working against you, diet alone can struggle to rebuild depleted stores. Repletion is a different task from maintenance, and it’s the point at which supplementation enters the conversation.

It enters through a doctor, though. Not through this article and not through an Amazon search. The reason is the one at the top of this page: iron is genuinely dangerous in excess, it accumulates, and hereditary haemochromatosis exists in people who have no idea they have it. Supplementing without knowing your numbers is a gamble with no upside, because if you’re not deficient you get nothing except the risk.

Iron Across the Female Lifespan

Requirements for iron for women are not static, and the direction of travel surprises people.

Through your reproductive years, monthly losses keep demand high and heavy bleeding pushes it higher still. Pregnancy raises requirements substantially, and that is firmly a clinical conversation with your obstetric team rather than something to manage independently.

After menopause, monthly losses stop and iron requirements drop sharply, down to roughly the same level as men. This is the part that catches people out: a postmenopausal woman who keeps taking the supplement that suited her at 35 is accumulating iron she no longer loses. What to Actually Do

Short version.

Eat the food. Getting iron for women right starts on the plate: heme sources if you eat animal products, non-heme paired with vitamin C if you don’t. Move tea and coffee away from your iron-rich meals. That costs nothing and helps everyone.

If you have symptoms, heavy periods, or both, ask for a ferritin test rather than accepting a full blood count as the whole answer. Get the number, not the adjective.

Do not supplement on suspicion. If the number comes back low, work out the cause with your doctor rather than just patching the symptom, because heavy bleeding and absorption problems both have treatments of their own.
